My Best Me Health is not an emergency service. If you are experiencing a medical emergency, call 911 or seek immediate emergency medical care. If you are experiencing urgent symptoms requiring prompt evaluation, please contact your PCP, visit an urgent care center, or seek emergency medical attention as appropriate.
We Are Not a Substitute for Primary Care
Patients are expected to maintain an ongoing relationship with a Primary Care Provider (PCP).
While we may evaluate symptoms, review laboratory testing, make recommendations, and provide treatment within our scope of practice, we do not replace comprehensive primary care services, annual preventive care, urgent care, emergency care, routine health screenings, or management that requires ongoing in-person evaluation.
Maintaining a relationship with a PCP is an important part of ensuring comprehensive, coordinated healthcare.
Specialist Care Remains Important
Depending on your individual health needs, you may benefit from ongoing care with specialists such as:
- Gynecologists
- Endocrinologists
- Mental health professionals
- Gastroenterologists
- Cardiologists
- Sleep medicine specialists
- Other healthcare providers involved in your care
We encourage patients to maintain these relationships when appropriate and may recommend consultation, follow-up, or co-management with specialists as part of a comprehensive care plan.
Collaborative Care
Healthcare works best when providers work together.
Whenever possible, patients are encouraged to share relevant medical records, laboratory results, imaging studies, consultation notes, and treatment plans from other healthcare providers. We may also recommend discussing our recommendations with your PCP or specialists to help ensure coordinated and informed care.
Accuracy and Completeness of Information
The quality of medical recommendations depends on the accuracy and completeness of the information available to us.
Our assessments, recommendations, and treatment plans are based on the information you provide, including your medical history, symptoms, medications, supplements, laboratory results, imaging studies, and other health records available at the time of your visit.
Patients are responsible for providing complete, accurate, and up-to-date information and for informing us of any changes to their medical history, diagnoses, medications, allergies, symptoms, or treatment received from other healthcare providers.
Incomplete, inaccurate, delayed, or omitted information may affect clinical decision-making, recommendations, diagnoses, and treatment plans. Whenever possible, patients should provide relevant records and test results to support safe, informed, and coordinated care.
Patient Responsibilities and Follow-Up
Successful healthcare depends on active participation from both the patient and provider.
Patients are responsible for following through with recommended laboratory testing, imaging studies, consultations, referrals, treatment plans, and follow-up appointments. Failure to complete recommended evaluations or provide requested information may limit our ability to make clinical recommendations, monitor progress, prescribe medications, or provide ongoing care.
Patients are also responsible for reviewing communications, test results, and recommendations provided through the patient portal or other approved communication methods and for notifying us of significant changes in their health status.
Timely communication and active participation are important components of safe, effective, and collaborative care.
My Best Me Health reserves the right to defer treatment recommendations, prescription decisions, laboratory orders, or ongoing care when clinically necessary information, recommended testing, specialist consultation, or appropriate follow-up has not been completed.
Treatment Risks and Patient Responsibilities
All medical treatments, medications, supplements, lifestyle interventions, laboratory testing, and healthcare recommendations involve potential risks, benefits, limitations, and uncertainties.
Patients will be informed of material risks, benefits, alternatives, and potential side effects when appropriate. By choosing to proceed with a recommended treatment plan, patients acknowledge that no treatment can be guaranteed to be free from side effects, adverse reactions, unexpected outcomes, or complications.
Patients are responsible for following treatment instructions, completing recommended monitoring, obtaining recommended laboratory testing, attending follow-up appointments, and promptly reporting new symptoms, side effects, adverse reactions, or significant changes in health status.
Failure to follow treatment recommendations, complete recommended testing, attend follow-up appointments, disclose relevant medical information, or report concerning symptoms may affect treatment outcomes and may limit our ability to safely provide ongoing care.
Patients acknowledge that healthcare outcomes depend upon many factors, including individual biology, adherence to treatment recommendations, pre-existing medical conditions, information available at the time of care, and circumstances beyond the control of the provider.
Limitations of Telemedicine
Telemedicine offers convenience and accessibility but has inherent limitations.
Because visits are conducted remotely, we may not be able to perform elements of a physical examination that could be important for diagnosis or treatment decisions. Certain symptoms, conditions, or concerns may require in-person evaluation, laboratory testing, imaging studies, urgent care, emergency care, or specialist consultation.
Recommendations provided through telemedicine are based on the information available at the time of the visit and should not be considered a substitute for appropriate in-person medical evaluation when clinically indicated.
Prescribing Limitations and State Regulations
Prescribing regulations vary by state and may change over time.
The availability of certain medications, treatment options, laboratory testing, and prescribing practices may be subject to state-specific laws, professional regulations, licensing requirements, pharmacy policies, insurance requirements, and clinical judgment.
As a telemedicine practice, My Best Me Health can only provide care and prescribe medications in states where the provider is appropriately licensed and where such care is permitted by applicable laws and regulations.
Not all medications, treatments, laboratory tests, or recommendations will be appropriate for every patient or available in every state. In some cases, an in-person evaluation, additional testing, consultation with another healthcare provider, or referral to a local clinician may be required before certain treatments can be considered.
All prescribing decisions are made on an individual basis after consideration of medical history, clinical appropriateness, patient safety, applicable regulations, and current standards of care.
Mental Wellness and Psychiatric Care
Emotional wellness is an important component of overall health and may be addressed as part of a comprehensive functional medicine approach.
However, My Best Me Health is not a psychiatric practice.
Patients with complex psychiatric conditions, active suicidal thoughts, psychiatric emergencies, severe mental illness, substance use disorders requiring specialized treatment, or needs that require specialized psychiatric management should establish care with an appropriately qualified mental health professional or psychiatric provider.
Functional Medicine Testing and Recommendations
Functional medicine testing and treatment recommendations are intended to complement — not replace — evidence-based medical care.
Recommendations may evolve as new information becomes available, additional testing is completed, or health conditions change over time.
Healthcare decisions should always be made within the context of each patient's complete medical history, current clinical condition, preferences, and goals.
No Guarantee of Outcomes
Medicine is complex and individualized.
Patients understand that medicine is not an exact science and that all treatments carry potential risks and uncertainties. While every effort is made to provide safe, evidence-informed care, no specific outcome, benefit, diagnosis, improvement, or response to treatment can be guaranteed.
Response to treatment varies from person to person, and healthcare plans may require adjustment over time.
